The extensive collection of resources includes interactive health tutorials, a medical encyclopedia, and health news pages that are continually being updated.
At Orthopedic & TMJ Physical Therapy Center our goal is to make your visit with us an enjoyable part of your day. Print the forms, fill out, and bring with you to your appointment. You taking the time before your appointment will enable us to process your paperwork more swiftly. Heather received a B.S. degree in Physical Education from Portland State University in 1976.
During the initial evaluation, your therapist will take your history, make an assessment, and design an individualized treatment plan that includes goals, frequency, and duration. Please complete your paperwork before arriving and allow 15 minutes for processing. If you did not fill it out, please arrive 30 minutes early.
Orthopedic and TMJ Physical Therapy Center is contracted with the following insurance networks:. Medicare - Traditional & Advantage, but Providence only if plan has Out-of-Network Benefits. PacificSource Health Plans, EBMS, No TMJ coverage, Legacy Health Plan-Call to check benefits. Please note that insurances accepted are subject to change.

My dentist referred me to Rachel Sobczak for TMJ massage to help manage the jaw and related headache pain I was having. Rachel was knowledgable, thorough and very personable. In two sessions she taught me techniques to help me manage the problem myself and it has been 100% successful. I was having pretty severe headaches for a year and now am pain free so I am very impressed!

I came in with severe jaw pain and inability to open my mouth fully. After Dr. Rachel did a thorough explanation about the jaw and muscles surrounding it I had a better understanding of how to manage my tmd. She also did some jaw release exercises and by the end of our session I was nearly pain free and could open my mouth! Great team of doctors. Definitely recommend

I highly recommend this clinic and the absolutely incredible providers I have worked with so far. Rachel is amazing, extremely intelligent with the ability to thoroughly explain and breakdown my treatment. Cindy also treated me with a perfectly gentle, methodical and caring practice that was helpful and soothing to my discomfort upon arriving. These ladies are all amazing. I saw all three for my TMJ and they helped immensely. I went in for TMJ when I could barely get my pinky finger between my teeth. After seeing them, I was able to eat semi normally. it actually turned out (after getting MRI later) that I needed more intense care from a TMJ dental specialist, but they helped me so so much until seeing the dentist. I highly recommend this place!

This clinic is a deep well of experience and knowledge. I received treatment over 10 years ago with Carol and am still going strong. I also highly recommend Lisa Graves, the best massage therapist I've ever worked with, as she too is located in this clinic. The women at Orthopedic & TMJ are a wonderful and valuable team of talented, resourceful, highly experienced medical professionals.
