Welcome to Meadow Park Rehabilitation & Healthcare Center, a warm and welcoming 134-bed skilled nursing facility serving the community's healthcare needs with distinction. Today, under new ownership, we are excited to infuse our nursing home community with Marquis' progressive rehab approach.

Dedicated to the post-hospitalization care of individuals recovering from joint replacement, cardiac, pulmonary, neurological, and other acute medical conditions, our excellent inpatient rehab program features physiatry driven rehab programming, daily therapies, state-of-the-art equipment and an array of amenities to enhance your comfort.

90-year-old male patient of hospitalist admitted to Meadow Park Healthcare and Rehabilitation Center from St. Agnes Hospital. Patient Admitted with Generalized Weakness, S/P Fall and Interrogation of newly placed AICD (4 days earlier) reveals Device Fired due to Rapid Atrial Rate.

Reviewed weekly with our in-house care team during Cardiac IDT led by Cardiologist Dr. Chopra; Dr. Chopra adjusted and Ultimately Discontinued Diuretic Therapy based on lab results Patient also followed by in house Physiatrist, Dr. Lepre while at Meadow Park.Upon admission, Patient required maximum assistance with all self-care tasks and was able to ambulate 10 feet with roller walker and maximum assist.
Our physician-led program is focused on helping patients achieve the most active life possible despite any cardiopulmonary diagnoses and physical limitations. We tailor our program for individuals with heart disease and pulmonary disease, with diagnoses such as CHF, COPD, post heart and lung transplants, emphysema, acute myocardial infarctions, and acute respiratory failure.
The clinical professionals at Meadow Park are trained to provide specialized care and services to patients with Chronic Kidney Disease and related diagnoses. Preserving kidney function and preventing further decline are the primary objectives of our Chronic Kidney Disease Management program. When caught early it is possible to slow, stop, or even reverse CKD depending on the cause.

Do not put your loved one in this facility!! My mother declined greatly in this place. There are maybe 4 people in this facility that actually care and do their job. I had to call 911 because my mother looked very ill. She is now in a hospital getting proper care. She had very low blood pressure ( and is on medication for high blood pressure). A UTI (which she had been treated for in the hospital before coming to this facility) and a C-diff infection. God only knows what would of happened had I not called 911. If you are looking to get good medical care along with PT and OT this is not the place

I would recommend Meadow Park to my friends and family. Therapy has helped me get stronger and the nurses have been very attentive to my needs. The food is great, and the facility is always clean. I have made so many friends here!

First of all the account name is Kathy Thompson but should say Kathy Schiaffino! Please note this. I just got home from Meadow Park after a month of rehab after a terrible broken leg. My care was phenomenal! I can’t begin to express how much I appreciated Mary who took the best care of me from day one and was just so good to me! Also Mrs Yolanda and Mrs. Martha thank you! Now on to physical therapy! I can’t thank Kathy Davis enough who motivated me and got me moving! She is the best! Thank you for the reminding me I could do it!! My occupational therapist Mo was amazing as well and I appreciate

My aunt, who is elderly has been a patient at your facility since November 27th. Your facility is very under staffed. She is in isolation due to an infection. Her very first day there at night, she pushed her call bell needing help and no nurses came. There were patients yelling for help but noone came. My mother tried calling the main number but no one was picking up the phone. The next day when I came to visit, I saw 1 nurse in the 5 hours from 1-6pm while I was there to visit to check on her or see if she needed anything. My aunt had had only 1 cup of water that was from breakfast that was warm

When my wife at the hospital, she couldn't walk. Since her 2 weeks of being at Meadow Park, her communication, mobility, and overall health have progressed. The staff at Meadow Park treated us like family. The facility is clean, neat, and an environment that provides comfort and dignity. Thank you, Meadow Park, for giving my wife the care she needed. I recommend this facility to everyone who needs rehab. We are so grateful!

This facility is very understaffed. The administration is extremely hard to get on the phone. Progress reports for patients are non existent. Do not go here unless you are able to take care of yourself. Some of the staff are very good but they are few and far between.

I am very thankful for everyone at Meadow Park. The aide Marcia and nurses Franklin and Bruce were amazing. I am grateful for the therapy department for helping me get back on my feet and I would give them a 10 star. Kim, Katie, and Cindy from therapy were very helpful. Two weeks ago I came to Meadow Park and couldn’t walk, but with the help of the Meadow Park Team, I now can. Thank you and I am very grateful!

On behalf of my family I thank Meadow Park Rehabilitation and Healthcare Care Team for extraordinary care my brother received. The attention given to his needs helped with his fast recovery to return home. We are grateful for the services and health care he received. Apart from quality patient care, the staff treat patient’s family with respect; answered question of health concerns within the limit of the staff then referred the family to right authority staff for further service. I will surely recommend this facility to friends or relatives that need rehabilitation care.
