Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor. We will notify you via email or phone when your pet's prescription is approved and ready to be picked up.

We have been doctoring at Countryside Animal Hospital with Dr. Jeff for the past three years. He is an extraordinary Vet. He did laser eye surgery on our dog after a local vet had done two surgeries that did not work. I was astonished at how my dog’s eye looked after surgery and how quick she recovered. He treats your pet with a lot of kindness. My dog now goes into the vet clinic with our her tail wagging and she is happy to see the staff. Dr Jeff has always taken time with our dog and is such a great teacher! Trish, who runs the clinic is extremely intelligent. When you call she can answer

I can’t thank this place enough!!! My cat had a tooth that had become long, and she couldn’t eat or drink. She is 13. This place let me bring her in at a moment’s notice, when 2 other places turned her down, and one said they couldn’t get her in for a week!!! Dr. Jeff seems very knowledgeable, and the staff is very awesome!! They were so good to… her. They most likely saved my cats life, as I don’t know what she would have done without food or water for a week. I will most likely be switching veterinarians, and will bring my other cats here also!!!! Love ❤

So responsive to my first email and continued excellent communication. I was lucky enough to find Dr Mills several years ago, he treated our Samoyed who was suffering from constant ear infections. This was the first clinic that ask"what infection does she have". One round of the correct antibiotics and she was good. Our current situation had our son bringing multiple dogs home. The first went to Countryside for years. But when the second dog followed we needed to cut expenses. . Now both dogs are back at Country side. We did allergy testing. It was quick and actually inexpensive compared to what

The service here is above and beyond amazing! I really can't thank them enough for fitting my 10.5 year old boxer in so quickly for a mass removal. The bond I have with my dog is important to me and it is important to them. They treated him with the love and kindness I would have. I highly recommend them and will sing their praises to anyone that will listen. Thank you so much!

Living in Illinois, I had a phone conference with Dr. Mills concerning our almost 15 year old Irish setter. When any of our pets have serious health problems, I like to consult with Dr. Mills and my local vet who is also excellent. I highly recommend Dr. Mills and feel that he is an outstanding vet. His knowledge and skills rise above that of most vets and I always value and am confident in his advice/recommendations.

Dr. Jeff, Tricia, and Emily are fantastic! Our little kitten had some tummy problems a few weeks ago and they took great care of him. They even called back to check up on him a few days later. Then when I had to go take the cat to the local emergency vet shortly after that, Dr. Jeff followed up the next day, gave me tips and pointers, recommended a… new food, and overall just reassured me. I just picked the cat up from being fixed, and was given very clear and well explained instructions for his recovery. I would highly recommend this veterinarian!!
