Our short-term rehabilitation oor is designed for individuals who require specialized rehabilitation and nursing services following illness or injury to regain their health and functioning. Our comprehensive team of rehabilitation professionals includes physical, occupational, speech and respiratory therapists.
We place great emphasis on a coordinated approach to care and open communication between staff, patients and families, to achieve the most successful outcome. We also have an outpatient therapy department for patients to continue their therapy following discharge.

A skilled nursing facility is only as good as its staff and caring for the elderly requires dedicated, qualified and professional personnel. Branford Hills Health Care Center is a community of caring. To provide care that will assist its residents to achieve an optimal level of functioning, independence and quality of life with out interference from pain.
The Memorial Trust Fund was founded as a non-profit, tax-free organization by Branford Hills Health Care Center in 1983. It was formed under Section 501(c)(3) of the Internal Revenue Code wherein all contributions are deductible as "Charitable Contributions" on the donor's federal income tax return.
Ledgewood Rehabilitation is a short term rehabilitation unit within Branford Hills Health Care Center. Ledgewood is a separate and dedicated unit designed for those individuals who no longer require acute hospital care but are unable to return home without additional care and services. Ledgewood provides the necessary rehab and medical programs to help patients reach their maximum potential and return home to their families and communities.
Serving impaired or physically challenged individuals, those recovering from illness or injury, or anyone needing the services of a companion or homemaker within their own home. When the time comes that you or a loved one feels the need for assistance and companionship at home, call on us for all the help you need.

My mom was a resident here for 4 months . I can’t say more about the staff and how clean it is kept. They treated us like family and took care of my mom very well. They took such good care of her that she is well enough to come home as she was able to access physical therapy most days. They also have recreation events for the residents and all the supporting people answered all our questions. Even during the covid outbreak they gave us an update and took every precaution. We made a lot of friends there and I am grateful that I could put the trust in the staff to take care of the great great grandma

I just called this facility to ask if I may speak with a resident. The front desk person was polite but was unable to assist me. She then suggested I speak with a supervisor I readily agreed and stayed on the line while she transferred me. An unknown female who did not identify herself answered the phone and as soon as I identified myself and asked if I may speak with my friend she became very abrasive and told me, “I should be calling the front desk”. As I tried to tell her I was just transferred to her by the desk” she interrupted mid sentence and more sternly told me to call the front

I've been here a few times and always got a warm welcome when I signed in and directions to the room I had to go to and found the bathrooms extremely clean. I enjoyed the artwork and photos in the hallways. The staff is friendly and courteous and enjoyed the calendar events in the elevator for that month.
