Each member of our outstanding staff, from our nursing staff and rehab therapists to our social workers, activity directors, meal preparation and housekeeping staff, contribute to the well-being and quality of life of our patients and residents. The Plaza Health Network Foundation plays a critical role for the five award-winning, not-for-profit rehabilitation and skilled nursing centers of the Plaza Health Network.

For patients and residents, the mission of the Foundation is to provide funds that allow the development of innovative, patient-focused programs and services that go above and beyond the basics covered by Medicare and Medicaid. For our community, we provide education, support and resources that benefit seniors and their families during each phase of the aging process.
Services
Plaza Health Network was born in 1954, when the Caring for the Sick Committee at Beth Jacob congregation of Miami Beach (3rd Street & Washington Avenue) founded a 12-resident convalescent home serving World War II and Korean War veterans, as well as elderly Jewish people. It was located right behind the sanctuary on Collins Avenue.
All Plaza Health Network centers provide the highest level of sub-acute care following hospital discharge so that high risk patients and those with complex medical conditions can return to optimal health in the shortest possible time. Our state-of-the-art telemedicine and telemetry program allows for immediate on-site intervention, reducing hospital re-admissions.
The APRNs at Plaza Health Network Rehabilitation & Skilled Nursing Centers are part of an interdisciplinary team that work to provide high-quality, cost-effective, personalized health care with a focus on health promotion and disease prevention, and an emphasis on both care and cure.

Their acquired expert knowledge base, complex decision-making skills and clinical competencies bring a comprehensive perspective to health care at Plaza Health Network's rehabilitation and skilled nursing centers.Advanced Practice Registered Nurses (APRNs) are prepared by education and certification to assess, diagnose, and treat acute illness and chronic conditions, including the prescription of medications.
As part of our ongoing mission to provide high quality, sub-acute care to adults and seniors in the South Florida community, we offer an advanced, inpatient respiratory care program that provides treatment, rehabilitation and education to patients with complex respiratory conditions.

Our treatment plans are designed for patients with chronic obstructive pulmonary disease (COPD), pneumonia, lung disease and those who are ventilator or tracheostomy dependent.Our goal is for patients to acquire the skills needed for greater independence and self-care at home.
Plaza Health Network and Jackson Health System have collaborated to provide clinically complex patients with an option for sub-acute care recovery including transplant and LVAD patients. Patients who require extra care and observation but do not have to be in a hospital setting can continue their recovery at one of Plaza Health Network's rehabilitation and skilled nursing facilities.
