Premier Physical Therapy & Sports Performance is an orthopedic rehabilitation facility with two locations in Las Vegas, NV that was founded on the principles of Faith, Fellowship and Servanthood. We believe that quality orthopedic rehabilitation is the result of sound education, innovative application, and most importantly, genuine care.
I need a referral to see a physical therapist: Nevada is a "direct access" state which means a physician's referral is not required to attend PT. However, a referral may be required by your insurance company. Call the Premier PT team and we will check your benefits. Physical therapy is painful: Physical therapists seek to minimize your pain and discomfort-including chronic or long-term pain.
Files available for download below. Forms are also provided in Spanish for your convenience. We are also in partnership with Rod Stewart DPT and Fallon Physical Therapy so we may be listed under one of these names on your insurance company's preferred providers list. As a courtesy to you, we call and verify your insurance benefits prior to your initial evaluation so there are no surprises.
