Direct Primary Care is a health care delivery model that gives patients access to health care at an affordable cost. Evolution Health & Wellness does not accept insurance of any kind, however, we will see patients at an affordable cost. Patients have flexible options to pay either monthly membership fee or per visit.
We accept patients 5 years of age and older. We do not currently provide routine vaccinations for our patients; however, we will work with you to arrange necessary vaccines. Direct Primary Care (DPC) is an innovative alternative payment model for primary care being embraced by patients, practitioners, employers, payers and policymakers across the United States.

Nurse Mason. Is awesome! Very knowledgeable. She answered all my questions plus more. She has helped me become a healthier me. She is very patient and you can tell she really cares. She do check-ups to make sure everything is going well. I will definitely be recommending her to friends and family. This will not be the last time Nurse Mason sees me!

Nurse Mason was very amazing. I don't like going to the doctor and hate taking meds. She made me feel extra comfortable with opening up about my health problems and gave a very detailed plan on how to get back on track. This is exactly what I needed and I look forward to getting back healthy with her help!!

Literally THE most comfortable I've ever seen my husband in a Practitioner's office! He HATES going to the doctor, he's always guarded, feels heavily judged, and isn't offered any real solutions for his health. Nurse Jasmine has facilitated a "no judgement" policy with him that has him extremely motivated! I can't wait to come back and update on his health progression while under her care!

I am not originally from Houston and was really stressing about finding a provider but I am glad that I chose this one! I was able to get an appointment in a timely manner and just loved the atmosphere in the clinic. I always enjoy my visits to NP Jasmine’s office. My chats with her during office visits are always uplifting and encouraging. She is caring and interested in how to help you gain control of your health. I appreciate the health tips that she shares in our visits as well as on social media. Nurse Mason is kind, professional and very detailed. I felt heard and she really took her time to discuss my condition with me. After my initial appointment she routinely checked on me to monitor my progress which was a pleasant surprise. I've never experienced that before. Highly recommend her, she truly cares about your well- being and health from all aspects.

I went in to see Jasmine thinking I knew a little about what I needed but she gave me different options and advice on what would be best for my situation, as well as some helpful tips to take home and read. I also received a B12 shot while I was there and didn’t feel a thing.. which may be small but it was super important to me!

I have been struggling with losing weight for some time now. I decided to give this place a try. Mason stepped in and helped me right away. She explained everything and went over all my options and let's just say the weight is finally falling off. She even checks on her patients to make sure that they are doing ok after the appointment. I am so glad I gave this place a try. Very affordable and has a great attitude. I would highly recommend.
