Staff not knowledgeable about patients well being . Very short handed on staff. Staffing are always on phones FaceTiming while dealing with patients that’s a invade of their privacy. Parking for visitors is always occupied by employees. Was told by an employee that there where’d told that they couldn’t park there so until so she wasn’t moving. It clearly posted visitors parking!!! I visit Several times a week and this is always a problem 😡 😡😡😡😡

My father was at the facility for two years! He was admitted to the hospital 3 weeks ago with a blood sugar over a 1000. The hospital said the shape my dad came in was from not being properly cared for. He was dehydrated the facility not only lied to us about his blood sugar the hospital had a hard time getting his medication list. Not one person reached out to us he almost died because of lack of care that he got at paradigm caused him now have a trach and feeding tube. I received a phone call today asking am I’m picking up his belongings which obviously the lack of knowledge knows I do not

This is a Nursing and Rehabilitation facility,
I think it is a great place for your loved one.
I'm also a Ministerial volunteer to the facility.
Many of the staff members are new and there's a new Management team altogether.
I have noticed nothing but loving kindness, respect towards the residents there at Paradigm Woodwind Lakes. I gave this facility a (five-star) rating! This is not my opinion during my tenure there, but of a truth!

I have a family member there and our family experience with the staff is not good by no means. The staff is not completely honest how our love is doing. They When you do get an appointment to visit your love the staff stands there the whole time like they are trying to hide something. Then they tell you have an hour but tried to cut it short. Our love health has clearly deteriorated in the several months she has been there.
