Louis H. Rappoport, MD is a Board Certified Orthopaedic Surgeon in Phoenix, AZ. He graduated from the University of Arizona College of Medicine in 1986 and has been in practice since that time. He completed a general surgery internship at Huntington Memorial Hospital. He then completed an orthopaedic surgery residency program at the Hospital for Special Surgery.

Arizona Spine Consultants accepts most networks within the insurances listed, however it is recommended that you call your insurance and give them our information to find out your coverage benefit information. Some insurances require a referral or authorization number to see our provider. Please be sure you have that information from your primary care provider prior for your appointment.
A copay (or copayment) is a flat fee that you pay on the spot each time you go to your doctor. Let's say you hurt your back and go see your provider. The amount you pay for that visit is your copay. Your copay amount is printed right on your insurance card. Copays cover your portion of the cost of a doctor's visit.
