Functional Assessment Rehab started in 1986 as Dell C. Felix, PT & Associates and in 2007 became Functional Assessment Rehab. We have focused on the need to provide a service to the community that focuses on functional abilities and return-to-work options. Our Physical Therapy Group is Utah's only physical therapy clinic dedicated to assessing and improving work function in the industrial setting.

As a physical therapist in Grand Junction, Colorado, Dell Felix was involved with sports medicine with a small college and a small osteopathic community hospital. One fall afternoon, in 1984, when leaving the locker room at the end of half time, an acquaintance who was the manager of a coal mine stopped him and asked I dont understand it.
On-site therapy at the employers location for physical therapy services such as: therapy for injured workers, ergonomic assessments, fit-for-duty assessments, job analysis, and consultation on injuries with workers and management. In-clinic therapy services can be focused on work specific activities to get your injured employee back to work quicker.
Do I need to come early? Attending the appointment at the set time is expected. There is paperwork to complete, but that is part of the assessment that you and the evaluator will review together. How do I get a copy of my report? In cases involving employment, workers compensation, or legal counsel, we are not able to release the report to any other individual except those who requested the evaluation.
