We can also mail forms to you if you'd prefer. Please call our office to make arrangements. If you are unable to do so, please arrive 15 minutes prior to your appointment to fill out the necessary paperwork. At Arrowhead Family Dental, we believe in the value of education. We have been offering graduating seniors at the Rice Lake High School and incoming students at WITC in Rice Lake the opportunity to apply for scholarships for many years. Our office has been fortunate to be able to help many students with funding their higher education.

Just visiting I was referred to this dental office. The courtesy extended was so wonderful.
The dentist was personable and honest with timelines for my stay here in Wisconsin. He could have certainly taken advantage of my temporary visit here but offered several options to me, not in his office’s best interest for profits mind you.
