Home health care is often recommended following an acute inpatient or rehabilitation facility stay or if you have a medical condition, an injury, or have undergone surgery that requires ongoing teratment and assessment.Licensed professional staff provides direct care to you and teaching to you and/or your caregiver enabling you to recover and be as independent as possible in your home.
The patient-centered medical home (PCMH) is a coordinated, team-based approach to delivering your health care services. We are always striving to improve and invite our patients and their families to share their ideas and opinions.

Our primary care physicians are the hub of all relevant care needs of the patient and manage a team of care coordinators and other non-physician providers that not only coordinate care within the walls of our patient's homes, but seamlessly transfer patients across multiple entities that may include community care, labs, specialists and hospitals.
There are several ways to get needed home care. New programs are constantly being developed for people who don't have medical insurance, or whose medical benefits have been exhausted. In many cases, a home healthcare nurse instructs a family member - or members - on how to care for patients. They'll also provide family members with a list of community groups that are available to help them, either free of charge, or at nominal cost.
The residence can be a private home or assisted living facility.
