Services
Community Care and Rehab Center is a 162-bed skilled nursing facility located in Riverside, California. Are you or a loved one about to be discharged from the hospital but are not yet well enough to go home? Are additional nursing and rehabilitation care and support needed around the clock? If so, our experienced staff can help.
Our experienced therapy team is committed to providing superior physical, occupational and speech therapy in an environment conducive to healing. We provide individualized 24-hour skilled care and rehabilitation services in a comfortable and family-friendly environment. Social Services provide essential information, manage requests and concerns, help in the care planning and discharge planning for each patient.
You've got questions. We've got answers. We suggest using the common areas of the facility to meet with your loved one. If it is not possible to visit in one of the common areas, we suggest a limit of two to three people in a resident room at one time. You may reserve the use of the dining room for any special event (birthday, holidays, meals) in advance with the Activities Department.

Do not bring your loved ones here. Just don't. God forbid you try to visit after you get off from work because you'll probably be turned away although you have an appointment. I'm starting to think I'm being turned away because I'm not vaccinated. I had my N95 mask and haven't caught anything yet. Coming from another county. Not to mention that nobody seems to know anything about her appointments, etc. I also have a family that keeps me busy. I should feel 110% confident that my mother is in good hands. I do not. The only department that has always been kind, knowledgeable and flexible with me

If I could give less than 1 star I would. All staff is uncaring. Food is slop and unintentional. Have had to call ombudsmen to file report and will call Medicare. Have to have food delivered to me so I can eat at least once a day. I cannot afford over $100 week for that. Yet they bill Medicare for meals I am not being fed. 20 pounds in 2 weeks is WAY too much to loose too fast and I'm stuck here till first half of Dec at least. And have lost everything I own due to being stuck here.

Linda, the person that answers the phone and is in charge of things is the worst ever. Instead of getting a very important phone call to my dad she transferred me to his room where there is no phone and when I called back she was very short as well. This place needs new management because everyone I go in there there need my Covid card which they are supposed to have on file but never do. They also do not have all my father's medications he needs. He has heart problems and needs a water pill and 2 weeks later he has not gotten it. This place is horrible and needs to get looked at by the government

My mother was a patient here. She had a stroke and was transfered here for care and rehabilitation. The staff are not helpful at all. I would call to get updates from their supervisors or staff on my mom's progress and be placed on hold and transferred around until I hung up or the call dropped. My mom would video chat with me and have nobody come check in her. Multiple times she complained about being cold and having not enough blankets. I tried to schedule an appointment to visit her and they told me their earliest appointment was 9 days away. The staff at this facility do not care about the

I'm so grateful for them to take my dad who's a medi-cal patient. I worked with Carlos and Cindy, and they were a great help. they were all so nice to me and my sister, one time i got so hungry bec. i had to wait for their paperwork, i told them that i needed to step out bec. im diabetic and i needed to eat. and Cindy was nice enough to offer me a sandwich. i mean who does that right? but anyway, the staff are very nice as well. i'm so glad that they also call me on step by step basis with my dad, when he goes through admissions by testing him, if he's put to regular rooms, stuff like that. its

I have worked with community care for a few years. All departments from therapy, nursing, activities and social services have been amazing. They have a new Administror Andy who you might bump into at any time even 9pm! He has invested a lot of time in making sure this place is running with quality care. Andy is very involved and he truly cares for his residents and staff.

My Brother Bruce has been in the care there at Community Care and Rehabilitation for the past 9 months. His case has been complicated and the entire staff including nurses, administrators, Social Workers, and those in the Business Office have been such a blessing to both my brother and myself. Navigating the best care for a family member to include ensuring they are covered under Medi-Cal, getting the right treatments, and understanding how each part of the systems work together is complicated and overwhelming.
