Please describe your spiritual life (believe in God or don't, am a Christian, attend church, believe in prayer, I'm not sure, etc.) Answer this question as well as you can: If you woke up in the morning and your problems miraculously vanished, what would life be like? How would you know the problem was gone?
Please list the names of ages of ALL those who live in the same household as the teen (siblings, other family members, etc.) How does your teen usually cope when under stress (tries to solve problem, becomes anxious, cries, shuts down, becomes angry, seeks out adult help, etc.)? Would you say your teen developed faster, slower, or at about the same rate as other children?
