We conduct 1:1 therapy individual therapy sessions that produce the best outcomes and will get you home faster. Complimentary medical transportation to follow-up appointments accompanied by our staff. Individualized discharge planning by our interdisciplinary team to ensure a smooth transition home or assistance with placement.
Yes, under certain circumstances. If a patient has been an inpatient in the hospital for three consecutive days within the last 30 days prior to nursing home admission, they would quality. However, the patient upon entering the nursing home must have a skilled need as defined by Medicare.
If both conditions are met, Medicare will pay the first 20 days in full and from day 21 to 100 the patient will be responsible to pay a co-pay (unless the patient has secondary insurance which will cover; check with your health insurance provider).If at any time during these 100 days the skilled service ceases, Medicare may no longer cover and payment either becomes private pay or Medicaid (if eligible).

If you are looking for a nursing home for a loved one stay away from Oasis Health & Rehab shown below. We had to resort to putting a loved one in there and had to pay 1 month in advance before she could be admitted. We were told that if the patient is not mobile we could visit her in the room otherwise they would bring them to the lobby for the visit. I went for my visit, they were all confused and after approximately 20 minutes they brought my friend out to the lobby in a bed hair a mess and barely dressed. She lived in there for 8 days and passed away.

My 95 year old mother was placed at Oasis for rehab following a series of strokes. After it was determined that she would need nursing home care, she was placed in ‘Hospice care’ and moved to another room. Their visitation policy is very strict and would allow 1 hour/day scheduled by the ‘Social Worker’. I live in another state and when I came down to visit my mother and mover her things out of her ALF, I was able to have this poor, near death woman brought to a room for me to see as I was not permitted to visit in room. I do Not work for this facility but I have been inside and from appearance, it’s tidy. However, I walked through the hall by multiple rooms one day and one room in particular caught my eye. From the hallway, I could see a patient’s feet on the ground. As I stepped in to check on the patient (who was not in my care as I do not work here), the patient was indeed completely on the ground & out of bed. I didn’t know how long he’d been there, based off appearance it looked like a little while, but I was the one who had to notify staff. That did not seem acceptable

I was hesitant to place mom into a nursing home because she wanted to stay at home for as long as she could. It got to the point where we could no longer keep her at home; therefore, we went searching.
We found Oasis. Oasis, it’s like the name states, I mean that they are amazing. It’s all about the people working there; from the front desk and the care givers, to maintenance. Everyone is very friendly, very pleasant and helpful.

A friend of our was sent there from the hospital for rehab she fell i,m sorry i ,i did not read all the review or i would have found some place she hated this place she would call every night wanting me to get her out of there at 90 she does not need he treatment she was getting so as soon as possible i got her out of there the front desk people sand staff are very rude she is very happy now where she is

I'm so happy and appreciative with the care my mom has received at Oasis. She has been here for almost two years now. Everyone that works here is so friendly, helpful, and knowledgeable. They always make me feel so welcomed and at home. The facility is always very clean and well maintained. The food is really good and tasty. They have a wide selection of menu options to choose from each day. Oasis has many fun daily activities for the residents to do. There is a beautiful courtyard that has a really nice gazebo, and a section with tables and chairs. If I ever needed a short or long term facility

My uncle had hip surgery a few months ago and had to go to a rehab I was a little hesitant due to the reviews but heard from a fellow coworker how good the care was at Oasis so I am so glad My family sent him there cause they have the most caring staff at Oasis. If someone didn’t know something they would direct me to someone who did. I never got a I don’t know like most places around! The activities department was so helpful with helping keep me and my family connected with my uncle. They were great about updating my aunt about how he was doing and kept us up to date on COVID and the precautions

Awful awful awful!!! Much like the other reviewer, I too would give this DUMP -100 Stars if I could!! I wish I read her review prior to my loved one going here. We both had very similar experiences. A year after her family member was there they didn't seem to do anything to make the place any better. DON'T EVEN THINK OF SENDING YOUR LOVED ONE TO THIS DIRTY SMELLY PLACE!! My loved one had to beg beg beg for a shower and a shave, which he never got the whole time he was there-luckily he was only there for a few days and we were able to transfer him out! Our mother arrived here from rehab and I have seen a marked improvement in the 3 weeks she has been at Oasis. They are very thorough in their ability to keep the residents risk free from Covid.
Mary in admissions made the transition effortless from beginning to end. The entire staff has gone out of their way to welcome our mother with cheer and support.
She has gained weight, gotten stronger with the physical therapy she has been receiving and is very happy with the care she has received. The communication has been adequate and I feel my mother is in good hands! Thank you to Oasis!

Thank you Oasis ! Our Dad has been there for a number of months now. Although this has our parents in two separate facilities, Oasis is beyond 5 star to my sister and I.
They were the first to lock down and were so right in doing so .
Everyone we work with, Christine , Gertrude , Lovey, Janey and our JESSY!! I know we are missing people and love you all ! He is NOT easy on a good day and Oasis "gets and understands" and they are great with him . They are constantly keeping us informed and on top of his needs .

My Mom was sent here for rehab after being very ill in the hospital. The Rehab was wonderful. Following rehab they worked with us to see if she might be able to transition to Assisted Living since she can no longer live on her own. Mom is in her mid-90s now. My brother and my husband and I all visited her there- with no advance warning- quite a few times over a period of several months. We live up in the New York/New Jersey area. My loved one was transferred from Delray Medical . I read what appeared to be stellar reviews. I was interviewed by Norisse who consistently reassured me once my loved one arrived he would be receiving the best care

I have nothing but great things to say about Oasis. My mother in law is in their care for a second time and I can not be more pleased. Last summer she was in their care for 2 months and she got out of there dressing herself, talking and walking, which is a far cry from how she went in there to begin with. She was malnourished, did not talk and could barely walk. When she was released, 2 months later she went back to her same living arrangements and 4 to 5 months after she was in the same state. My father stayed at Oasis for about six weeks during his recovery from a broken hip. John, my pop, is let's just say not the easiest person to please and/or get along with. He is somewhat of a hermit, not really a big fan of people in general and certainly not used to interacting with a lot of individuals. What a wonderful Health and Rehabilitation Center deserving of a 5 Star Review. After 2 months of being in the hospital, mom is in desperate need of rehab. Having complex medical needs just about every rehab center turned mom down with the exception of Oasis. They went out of their way to provide patient and family support rarely seen these days. Their Administration, PT, Activity Director, Nurses, front desk staff and security are all a cut above the rest. We are blessed knowing that mom is in great hands with everyone at Oasis. My husband was a patient here. This was the third rehab he was in, and I must say if you or your loved ones need this type of care this is where you want to be. The personal care, consideration concern and most important respect he was was given was exemplary. I was always contacted when there was any question. I was always part of his care planning, made to feel like it was a family rather than a body in a bed.
