Spring Creek Nursing & Rehab offers a wide range of quality care services designed to help our patients recover and return to their quality of life. Spring Creek Nursing and Rehab specializes in post-acute care for orthopedic procedures, stroke recovery, and cardiac procedures. We offer an extensive rehabilitation program consisting of physical therapy, occupational therapy, and speech therapy offered seven days a week.
Clothing that is comfortable and easy to put on and take off, and a comfortable pair of shoes (For precautionary purposes, please label all belongings with the resident's first and last name). Must undergo a minimum of a 60-day "wellness break" (not hospitalized or under "skilled care"). Must have a qualifying hospital stay (three consecutive, inpatient overnights in a hospital currently or within the last 30 days).

Spring Creek Nursing SNF can also assist you or your loved one in long-term care. If you are no longer able to live independently or need some daily assistance and nursing care, Spring Creek SNF is your solution.
