Great hospitality. Initially I wasn't sure if I would leave because I was in a poor medical condition. With the help of God and your staff I am able to return home back to my prior level of function. I am forever grateful. Thank you all. I enjoyed my time and everyone. My uncle has been there just 3 days, and the people there are so compassionate and kind, I honestly can't thank them enough.
They go out of their way to set up a meeting for you so you know everything that's going on with your loved one. This place showed me how my uncle should really be cared for. He was at a facility before, some odd months ago just briefly for 2 weeks if that, and the difference is just night and day.

Boulevard is a terrible place to send your loved ones. The building and especially the rooms are extremely filthy and grotesque. When you pass by the rooms in the hallways, the smells are unbearable. The patients are not cleaned properly and it is very apparent. Most of the nurses and CNA‘s and other staff are lazy, unprofessional, and rude. They lack people skills and are in a field that requires it. You hear the patients crying for help and they are ignored. Thank goodness I visited my loved one when I did because he was not doing well, and Boulevard was doing absolutely nothing to help him

I have called your facility multiple times to speak to the nurse regarding my patient for continuation of care. The patient's wife is unable to speak to staff to get updates on husband's health. The receptionist will not release the name of nurse. I am offered to leave a message for nurse. Days have gone by and no return call from nurse and I call again with no luck. In order to provide patients with the best care after being discharged is to have communication with the patient's pcp. I will place a formal complaint with the insurance company and insurance commissioner's office.

I had a family member there and they were horrible to her. She had a stroke which left her unable to use her right side! The night shift people were especially horrible. She needed to go to the bathroom and rang for ever till finally she could not hold it and wet herself in her bed waiting! She called out and no one came! She spent the whole night in a wet bed and hospital gown. Then next morning the “nurse” told her that she was dry and not to complain. They would put her in a chair and leave her there in a corner for four hours till it was food time. They lost her teeth (bridge) and her prescribed

Truly unorganized facility. You are told different things by different people. Reception person changes every day.
DaRyl , the social worker . who seems to be there for a check , does no return phone calls, even the receptionist could not get a hold of her , either on her cellphone or extension. She failed to coordinate the transfer to a dialysis clinic, got there, they had no paperwork on me. Dialysis to live is not something to play with, so to the ER I went. She promised a worker, nothing given on the day of discharge. She has an attitude when told of her mistake, WOW. A Revamp is needed, NOW

Rehab facilities are extremely difficult to gauge. One person will praise the location, the next will condemn it. I feel that some of the reviews are written by the employees or staff and also that some are written from spite. What suffers here are the needs of the people searching for a decent facility. Unfortunately there is no way of knowing for sure.

Boulevard Rehab Center is a beautiful facility that I’ve personally had the pleasure of working at for the past 3 weeks. The facility has some of the most caring and compassionate staff I’ve ever worked with. Boulevard Rehab has an amazing therapy department which offers Physical, Occupational, and Speech therapy that will get your loved back to optimal health. I highly recommend Boulevard Rehab Center for you or your loved one.

The aids have very thick accents, which would be fine if they would speak up! They have lousy attitudes and mumble answers (if they answer at all). This does not work when most of the patients are elderly and hard of hearing to begin with. They are terribly unresponsive. We rang for assistance for my mom one evening and waited 25 minutes before I walked to the desk and found 3 employees just sitting around chatting and ignoring all of the call lights in the wing. The nurses try their best but are severely understaffed.
