We do this by thoughtfully applying the highest quality, compassionate, medical staff with the best technology to provide you with extraordinary healthcare. Our medical team is here to provide that extra layer of care that some patients need.
We recognize that life does not revolve around standard business hours - neither should your healthcare. Titanium Healthcare now provides immediate after-hours Registered Nurse and Provider support for when your questions and medical needs can't wait until the clinic reopens. Any active Titanium patients can call their regular clinic phone number and select the after-hours options for support.
The extra layer of care you need. In addition to primary care, we're available to help you stay out of the hospital. During our post-acute program, you'll receive extra attention, extra support and extra care. We make sure that your questions are answered and that you get the help you need.

Our doctors will spend extended time with you to truly understand your health complications and to help you manage your chronic illness.

Titanium patients receive the best possible Primary Care. Whether you're coming to see us for just a simple check-up, or more complex conditions, our experienced healthcare team are here to take care of you. With our longer than average appointment times, our doctors can take the time to get to know you and your health history.
Enhanced care management. We're working with the Health Homes program (HHP) to provide enhanced care for our communities. The Health Homes Program provides extra services at no cost to people covered by Medi-Cal who have certain chronic conditions. The HHP provides extra support for free. When you join the HHP a care coordinator is assigned to you to make sure that you get the healthcare and community services you need.
