The mission of Harbor Haven Health & Rehabilitation is to serve the medical, psychological, social, and spiritual needs of each individual resident with a total commitment to quality care, carried out by our TEAM of dedicated and compassionate health care professionals. Other services such as dental, optometry, podiatry, beautician, pastoral care, recreational therapy, and social services.
Harbor Haven Health & Rehabilitation has a separately located short-term rehabilitation household called Island Harbor for those needing physical, occupational, and speech therapy and post-acute care. The Island Harbor household is dedicated to post-acute care, for those who are recuperating from conditions such as joint replacement, fracture, or stroke.
Harbor Haven Health & Rehabilitation offers outstanding and compassionate nursing services for those in need of long-term care. We have three newly remodeled units that are dedicated to providing long-term skilled nursing care--Meadow Harbor, Sunrise Harbor, and Sunset Harbor.
Many of the residents in these households are in our care due to chronic medical conditions associated with aging, such as congestive heart failure, diabetes, and decline in mobility.We also care for residents with long-term care needs associated with Alzheimer's, dementia, and other behavioral health issues in these households.

Snug Harbor is a newly remodeled, secured household, providing care and services to residents with dementia, who require a continuously structured and contained living environment. It is a unique care environment serving those who are at high risk for wandering or elopement, providing safety and comfort for those with these needs.
The staff of Harbor Haven Health & Rehabilitation are very pleased to announce that the facility was chosen to participate in the State of Wisconsin, Department of Health Services, Music and Memory Initiative. The Music and Memory Initiative is a music program designed to improve the lives of older adults, particularly those suffering from advanced forms of dementia.

My relative had an accident and needed more care than could not be done at home. Only about 2 weeks. After the first two days, they requested help to shower. In the two weeks there they never got a shower or bath. Something always came up when they asked. Being their injuries affected mobility, they needed assistance getting to the toilet. On average it would take staff 30 minutes to be available to assist. The staff is very friendly. The facility appears to be understaffed.

My husband, Gerald Marking, was in Harbor Haven beginning 12/31/2020, this facility had NO idea how to treat my husband's Type 1 diabetes, which his Pancreas no longer produces any insulin. Because of HH's Dr. Lisa Weber, the sliding scale I had used for his insulin units, he Never developed ketoacidosis which is a Medical Emergency, he was taken to the ER at St. Agnes ER, an ER Dr. told me my husband was a very sick man. My husband often had HIGH blood sugar numbers, nurses injected him with few units of Insulin. NO ONE at HH listened to me it was Critical a sliding scale for his Blood Sugar numbers
