My mother in law was transferred here from Kindred less than 3 days and now has to be transferred to the ICU. She cannot speak for herself. She was not supposed to have a room mate but she did. Nurses can’t answer why. Now she has pneumonia and is very sick. The transport team told what bad shape she is in and implored us to her to a specific hospital. Do not send your family here if you care about them and want them to get any better. There is no excuse for the condition of this place. I hope your joke of a facility is shut down and someone sues the pants off of you.Nothing to discuss with administration

Just pulled my 92 year old dad out of there. He had bed sores not treated!!! They let another resident attack him on several times. Took him straight to the E.R had a sevier U.t.i and dehiddrated and malnursment. He is is terrible shape now.!!! Would not give him his meds or money when we left. His catheter was leaking etc!!!

My husband is still at West Melbourne rehab They have the best physical therapy, occupational therapy speech therapy My husband had a stroke in July he has been at this facility for over a month now receiving occupational therapy physical therapy and speech therapy they are excellent they are doing a wonderful job If anyone has a loved one who wants to get physical therapy occupational therapy speech therapy I would 100% recommend this facility, the rehab is really really good, kiddos to Lisa the supervisor in the rehabilitation department! She has a strong Team!

My Dad has been there for a month. He has two weeks left. I have had several issues since he has been there. From food to care there has been something I have spoke to social services a few times . A few workers are good. Cindy is the best nurse you have on the C wing. Hedie is good to. They are the ones I see the most. Most of your CNAs don't want to help. Beds squeak and if squeeze the gray button for help it could be up to a hour .Disappointing

Very disappointing my stepfather has been here for a week he has he had yet to have a shower. I would not recommend this place.The nurse did not even know much about how to operate a Cpap machine. Zero stars! Sad to think that all patients are to stupid to know what’s going on. I will be reporting the services as poor. I came for myself and saw him dirty in his bed. Hope you read this and do something with your staff.

I have a close personal friend I have called Mama for the last 20 years as she is like a 2nd mother to me. She has recently been placed in this facility and calls me often complaining about not being fed, not getting her pain medication as prescribed, and not being treated like a human being. If I had any legal standing I would remove her immediately. From what she tells me she would be better off ANYWHERE else She claims there are bugs all over the place as well. I believe her I hope somebody will do something like hiding static cameras around the facility to document what is going on!!! Youtubers

Do not allow loved ones to be transferred to this facility. My father in law was being told that my mother in law was getting her therapy 3 times a day. Because of covid we could not go in to see what was going on. Every day I called her and she would let me know she had been alone all day. Someone came to put her in a chair and that is were she stayed for the day. She is now home and can no longer walk.

I’ve had an absolutely horrible experience here where my mother is residing. Due to COVID-19, we are only able to visit her through a window and by appointment only (which are only on weekdays, 9-5). Made an appointment last week to see her this morning, only to be called at our appointment time to be told it would not be possible to see her. No previous notice, just a phone call at the appointment time! I live over 90 minutes away and drove here with the sole purpose of being able to see her before beginning my workday. Their excuse was that because she was transferred to another room two days

This facility is deplorable. The furniture in the patient rooms is falling apart. Laminate peeling off the beds tables and other furniture. It looks as though it is sourced from the worst thrift store. The rooms are tiny, dingy and smelly. two people barely fit and two patients are in the room. As I walked around for a week the halls were littered with furniture stacked on top of each other, it is still that way. This is dangerous for these folks to be navigating these hall by them selves, bed mattresses on the floors. I saw a food tray from a patient half eaten sitting on a bed in the hall for
