Welcome to Hillside Nursing and Rehabilitation Center of Wake Forest and thank you for visiting our web site! Our mission is to provide all of our residents and their families with exceptional care, service, and support in order to enrich their lives in a respectful and dignified manner. We strive to provide dedicated, compassionate care to everyone who comes to us and treat everyone in an environment where Kindness Meets Caring.

Our nurses work closely with our doctors, therapist, social worker, activity director, and dietician to create an individualized care plan for each and every resident. We constantly focus our efforts to deliver excellent care with a warm, family-like, personalized manner.
Services
In 1992, a local family with several decades of experience in the field of long term care, constructed Hillside Nursing Center to provide quality and compassionate care for patients needing skilled nursing supervision. As our reputation and community trust grew, we later expanded the home from eighty to one hundred and fifty beds.
Hillside Nursing and Rehab offers extended nursing care in as homelike a setting as practical. Nursing care is available 24 hours per day, seven days per week. Our Social Workers support the social, emotional, and spiritual needs of our residents. A registered dietician oversees the menu and meals are prepared in accordance with each resident's dietary needs.
How much does it cost to stay at Hillside? This would depend upon whether you will pay privately, or if Medicare or Medicaid is to be billed, or whether you may have private insurance. How do you apply for Medicaid? While there are financial limitations applied to those who seek Medicaid assistance, we will provide you with some information and persons to contact at those offices.

10/2021: I AM IRRITATED & DISGUSTED right now!! I live in Wake Forest & my grandmother, who lives in VA, had a medical emergency that occurred which caused her to be hospitalized for almost 1 month. When the hospital was ready to discharge her to rehab, I told them about this place & they reached out to see if they could take care of her medical needs. They never responded. So, I called myself & was told Darlene handles that. I spoke to her and she told me that they don’t have beds for out of town people. ????? Huh???? She can no longer live in VA alone, so she would need to be where I am. STILL

My mom is currently a rehab patient in this facility. I transferred her from one that was truly neglectful. She has thrived. Everyone who works with my mom has been extremely professional. The therapist are kind and considerate of her individual needs. The communication between me and the medical team has been fantastic. They are on top of her litany of medical needs. They have great COVID response and share and communicate possible issues that arise. I am so thankful, having dealt with facilities that were terrible, to have my mom in a place that she feels safe, and that truly cares about her

Mt father is very vocal. If the conditions described in the negative reviews were accurate, he'd be yelling at the top of his lungs.
Everyone I've interacted with, from the front desk, to the admin office, to the nursing staff, to the PT specialists, have been wonderful.
My father has not in any way insinuated that the conditions in the facility are substandard.
I am very grateful he's there.

This review is based on my families recent experience at Hillside Rehab. PROS: the rehab staff was wonderful -- professional, experienced, and knowledgeable. The wound care nurse was especially good as was the OT and PT. When you are faced with finding immediate rehab care following a hospital stay, this is a good option if you live nearby and can be a patient advocate during the rehab stay. They have a liberal visitation policy compared to other facilities (9-5 during the day) which is one of the reasons why we chose it.
CONS: I would give communication a 0. There is no patient advocate other

Working here was terrible! This place has roaches and ants crawling on the residents. Also bed bugs. The residents are treating so unfairly. AIDS would yell and scream at the residents. There was a few times the aids would lay hands on the residents. Nurses did not bother to help cnas. Work ratio was terrible sometimes 38 residents per cna! I would not reccomend keeping your loved one here.

This place does not treat their good employees well at all. And the lady Kristy is a very disrespectful individual. I wouldn't let my dog stay here if you paid me. Roaches seem like they are in charge of the whole facility as they roam as freely. I would not eat here? This place is disgusting and needs a new director or someone who really cares for the residents and the employees. The roaches are getting better care than your love ones

My Mom went to Hillside after a fall with a broken hip and shoulder. We were so pleased with Hillside. Are they perfect? No, but the care and compassion my Mom got was amazing. Some of the other reviews stated that the rooms were roach infested. My Mom was in 3 different rooms (overflow on her first night there, rehab & skilled nursing) during her stay and I never saw one roach or bug of any kind. The staff treated her like she was one of their own. I even watched one nurse kiss my Mom on the forehead. And the rehab folks are outstanding!! Yes, people wander the halls, wandering the halls is good

I agree with others stating the appearance is misleading. My mom went in for rehab and she declined very fast. Not once did the doctor or Nurse Practitioner call me. During my last couple of visits my mom was barely eating or drinking and I can't imagine she was taking her medications even though they told me she was. Overall no one seemed overly concerned but me and acted like I was over reacting and seemed inconvenienced by my inquiries. I even hung around for 2 hours one morning to see the Nurse Practitioner visit my mom. I was told she knew I was there waiting but she never showed. When I left

My Grandma was a Resident at Hillside Nursing Center for the last year of her life. We lost her in April. The kindness and compassion that was given to both my Grandma and my family during this time will never be forgotten. No one is ever ready to admit their loved one into a Nursing Home nor is the Resident themselves ever truly ready but Hillside helped us each and everyday to make our decision to do so a lot easier. If a issue ever came about the staff made sure it was addressed and resolved quickly. I have had friends put loved ones into other facilities and by far Hillside has been the best

Let me start by saying my wife is a retired nurse, my son is a nurse and his wife is a nurse. As a point of reference my daughter had been a part owner of a consulting business which evaluated nursing homes. My wife came down with an infection on July 1 and was admitted the hospital on July7. When released she had become so weak that rehab was necessary and we selected Hillside Nursing & Rehab. For 9 days she was provided excellent care at Hillside and on July 16 the symptoms of the infection reappeared. I was so impressed with the care and treatment she was receiving, I told her nurse that if
