When you're making a decision about the right place for rehabilitation care, Escondido Post Acute Rehab should be at the top of your list. Here you will find answers to your questions about what to do first, how families play a role, and what to look for in a care center. We're large, so we have resources the small facilities lack.
Consequently, we have the unique ability to customize and accommodate the individual needs of our patients, from what you like to eat, to what activities you like to do-even the places you like to visit. From a short few weeks while in rehabilitation and recovery to perhaps longer-term residency, our goal is to make sure you feel safe, impeccably cared for, comfortable, and well accommodated during-and even after-your stay with us.

Services
In addition to specific therapies, we also offer enjoyable and convenient amenities, such as Wi-Fi, so if a family member needs internet access, it is always available. Should the patient want access to a computer, our activities room has a PC available, as well as numerous other activities to keep them engaged during their stay.
It often happens that when a loved one is hospitalized after a fall, a spell of illness, or a surgery, a decision is thrust upon the family to quickly find a skilled nursing facility to accommodate the rehabilitation process. If you choose Escondido Post Acute Rehab, know that we empathize. It's our goal to make the transition easy for our new patient, as well as keeping the family apprised of the progress they make while in our care.
You could think of our center as a community within our community - one that's focused on care and healing. When patients come to our facility, we work directly with them to get them rehabilitated as quickly as possible with loving care. Friendships are often started in our care center. Some people who have been in our care sometimes come back, simply to say hello to staff and other patients.
We focus on the special needs of our patients, uncovering likes and dislikes based on an in-depth patient profile that is gathered with the family. Nutritional needs are always taken into consideration, so we carefully review food allergies, dietary habits, favorites and preferences, always assuring that body weight is monitored and health is promoted.
At Escondido Post Acute Rehab, we want your patients to experience a smooth transition to post-acute care, without hassle or complaint. We are responsive, understanding, and flexible. We are aware that you have many skilled nursing facilities to choose from. We want to be your go-to local facility, the skilled nursing center you can rely on for the best rehab, the best nursing care, and the best skilled nursing care available today.

I promised to write a review. It is challenging to post a review on yelp. Great initial customer service on the phone. Rubi was very pleasant and quite accommodating. The employees were very pleasant and accommodating. The rooms are okay. This is not a resort. It is a place for rehabilitation and being kicked out of Palomar hospital on such a short notice, they took my father in with no hesitation and made him as comfortable for the short period he was there.

Unfortunately i have placed my trust in a very unprofessional staff and facility. In a difficult time my family was going through when our uncle was very ill and weak one of there so called "nurses" was supposed to be assisting him to the rest room he was a bit slow to move like many of the other patients , the" nurse " pushed and hurried him in a very impatient and rude way almost causing him to fall and making him feel unsafe . After he asked her to be more considerate (in spanish) she said to him "english ...SPEAK ENGLISH!". This is called abuse!!!! We deeply regret

If I could give this place a ZERO (0) I would. The 1 is only because of a few of the people that work there, that helped me after the fiasco with my mom! My mom had knee replacement surgery and was transferred there after her recovery at the hospital at the beginning of January 2016. From the moment my mom & I got there we were COMPLETELY ignored by the Head Nurse...who told me a few times that she was "overwhelmed" and she would get to us when she could...WHAT??? I personally witnessed this head nurse ignore other patients/residents as well. They were complaining to her about

The admissions process was seamless, and the coordinator was proactive and compassionate. The facility was clean, and the nurses were accommodating and warm. We would highly recommend this facility for end-of-life placement. We only used them for a few days but everyone from the administrator to the CNA was very involved in the care of our patient.

It was an amazing experience from the moment we walked in the door. We met with a lady named Rubi, she was so knowledgeable and had an answer for every question we had. Her attitude matched her level of knowledge "above and beyond". Thank you so much for giving us a tour of your wonderful facilities.

My mother and I had an awful experience there while picking up my grandfathers belongings. After he had passed away we drove 9 hours to the facility so that we could gather his belongings. When we arrived at the facility 24 hours after he passed away we had discovered that they put all of his things in trash bags and put those bags in an office. We specifically told the facility that we wanted to pack his belongings. As you can imagine finding his things in this way added to an already difficult time. All the facility could say is "we don't do things this way". Obviously changes
