Southwest Nursing & Rehabilitation was an upgrade from the Nursing facility in Burleson, however it still needs work and restructuring the staff! Therapy was amazing, however the Nursing part was not knowledgeable on the changes in patients,had I not been there for my mom she would have gone sepsis!
Infection set in ...in my mom's pic tube,the a UTI was overlooked and dehydration...but I caught it,before it got worse! 2 patients one being my mom was only changes once in an 8 hour period! I was there!!
When the state came in everyone became a priority, before that,call light buttons would go off

The therapy department at Southwest Nursing and Rehab is truly one of the best in Tarrant county. The rehab team is knowledgeable and dedicated to the therapeutic needs and personal goals of each individual patient. The Southwest rehab team has access to the latest techniques and technologies to provide interventions and modalities in one of the largest gyms in Ft Worth. My mother has been in this nursing facility for a very long time! This review might be different if Covid had not hit. However, because of Covid we are not allowed to visit. Leaving all the caring of our mom up to them! They took care of her like an abuser takes care of a dog! She had malnutrition and was severely dehydrated. Not to mention they didn’t take care of her teeth at all! She developed bed sores and probably had not had a bath for a long time! I hope they understand karma comes back around!

My Mom was here a couple of years ago and the experience was less than appealing - HOWEVER! My siblings and I toured and based on the changes that were presented by the representative, we decided to give them another try! I have to say, the care was GREAT, MUCH cleaner than it was before, the staff was friendly and the rehab team got our mom back to being independent and she is back home ALONE and doing well. They're undergoing a refurb and my understanding is that the new management has been there less than a year- they were worth a second chance.

My mother was in SW back in the late summer and early autumn of 2012. The care here was mixed but on the poor side. She worsened here after improving at the previous facility. We moved because the other facility did not offer private rooms except for rehab. The physical therapy at this other facility had done a lot to improve my mother's condition and I was hoping to continue her recovery following the damage done to her from a fall & subsequent medical malpractice at a local hospital. While we were at SW, my mother’s care tended to be poor & their administration was uncooperative to say the

Whenever I have a problem, the team takes care of it! I've lived at SW Nursing and Rehab for 6 months and I've made a lot of progress since I've been here. They gave me a home when I didn't have one and couldn't take care of myself. I could go somewhere else but I choose to stay here because the staff takes excellent care of me. I can be rolling in my chair and if someone sees I'm struggling, I don't even have to ask for help, they just do it. Ashu is my wound care nurse and he took care of my foot when I had my amputation. Julianna, one of my CNA's, she's always smiling! She always comes back
