My Chart offers you secure, easy-to-use and free online access to portions of your medical records. Your patient login allows you to view test results and health summary, request prescription renewals, access trusted health information resources, and communicate electronically and securely with your medical care team.
At your first visit, our front desk makes a copy of your insurance card to keep in your file. Please have your insurance card with you at your visit. If you change insurance plans, we'll update your record at your next visit. Certain insurance plans have provisions that require you to obtain referrals when specialty care is needed.
Your first visit includes a thorough evaluation by one of our therapists. This includes reviewing your medical history, testing to diagnose your condition and assessing your current functional ability level. After the evaluation, your physical therapist will discuss your customized treatment plan with you.
This group helps you prepare for and recover from weight loss surgery. It is a medically-supervised exercise program that includes checking vital signs, energy, pain, and more. The program helps you avoid injury, strengthen muscles, and increase movement. This exercise class helps you return to exercising following breast cancer treatment.
