Services
Periodic examinations are important to evaluate the health of your eyes, how clearly you see, and if you are having any difficulties with the visual demands of work, hobbies, or sports. You will be asked to fill out a history form prior to every examination, even when you think that nothing has changed since your last visit.
Your glasses are an important part of your vision care. It is important that your prescription be fabricated to provide clear, comfortable vision for all of your activities. It is also important for them to be attractive, durable, and fit comfortably. To help us serve you, please inform us of all of your optical needs for work, sports, hobbies, safety, and sun protection.
What to bring: A copy of your medications, insurance cards, completed questionnaire that was either mailed or you downloaded from the website, contact lens Rx and glasses you are currently wearing. If you have been referred to our office by another professional, please bring information with you, if possible, that explains why you have been referred for a consultation.
Do I need to bring all of my insurance information, including medical cards along with my vision coverage?. Yes, we require all current insurance information when you come into the office for the exam for optimum care. According to the insurance industry, it is the insurance members responsibility to know what their coverage consists of prior to any doctors appointment.
