Form & Fitness' mission is to offer unparalleled exercise experience to the people of Ozaukee County. We will provide 24 Hour access to the BEST training programs, the BEST nutrition programs, the BEST cardio and strength equipment, and many other quality amenities. Form and Fitness was voted the BEST health club in the Milwaukee Business Journal for the last 10 years.

Form & Fitness opened in 2001 by local exercise and physical therapy professional Dr. Ben Quist, DPT, CSCS. The business was created to shelter and nurture people who were seeking a more fit life, but who couldn't find the help and support they needed to be successful in traditional, mainstream membership clubs.
Form & Fitness was founded 16 years ago as a personal training company. Our mission remains to provide you with Southeastern Wisconsin's best personal training services. Appropriate Resistance Training: Resistance training forms the cornerstone of any good exercise program. Corrective/Preventative Training: F&F's correction and prevention component of training will address any problem areas you many have.
In 2006 Form & Fitness Health Club opened Wisconsin's FIRST speed, agility, and strength training facility for athletes. This state of the art facility features a certified training staff as well as the latest and best training equipment for strength, function, and performance enhancement training.
After I had my daughter at age 21, I realized I was not healthy and wasn't setting a good example. At first, I did strength training and cardio, which challenged me in different ways. One of the things that I do now is track my food and eat an 80/20 diet. Advice I would give my peers would be to START - just start!

Ample weights, machines, instructors and 24/7 access. The staff (especially Josh and Laura) both behind the desk and on the floor are awesome. Wheather its aerobics or strength training I never have to wait on equipment. There's even day care for children available while you exercise. Its THE place to go locally.

Silver Sneakers Yoga in the tent. Perfect. It's my first time back since Covid 19. They observe all required protocols. Even have a new HVAC system to clean the inside air. It's a good place to be.

This is the gym to go to. I don't write reviews often, but felt compelled to after my awesome experience here. I have not encountered another business in the local area that goes above and beyond to satisfy their customers. Every single employee I have worked with so far has been nothing but knowledgeable, considerate and professional. I received every detail I could need on membership and service options, and when I emailed with additional questions, they were answered immediately. The prices are also some of the best in the Milwaukee area (I left WAC to join Form & Fitness based on the services

Great gym. No seriously, Form & Fitness along with the YMCA are the best gyms in a 20 miles radius in my opinion. If they had a hot tub I would literally cancel my Y membership and only go here. They take amazing pride in the look and cleanliness of this location. I am super proud and happy to be a member.

Hands down the best gym in the area. Equipment is mostly new and updated, if anything breaks its fixed quickly, they are always adding or trying new things out which is nice to see and overall staff is great to work with.
Great value for the price too. For under $40 a month to have access to a facility such as this is a great deal. I highly recommend Form & Fitness to anyone looking to join.

Based on my a lot of gym experience over 10 years, I can say this gym is the one of best gym! Staffs are super nice and equipments are well maintained and organized. Especially they are opening for 24 hours!

I have been working out at Form and Fitness since the club opened and enjoy the 24-hour access and the welcoming environment. Recently, I needed advice following an injury and found it easy to get reliable information from the trainers and staff. They are passionate about what they do and care deeply about the members. Just like any organization, culture drives success and it is evident they want to create a positive environment. For a great price I not only get to work out with state of the art equipment but I also get to use a clean and great looking locker room that has showers and a steam room. There are also classes, and many to choose from. The trainers are so nice and so is the rest of the staff. The smoothie/coffee bar is great. Everything about form is great :)

Form and Fitness is a great health club. They have a variety of classes for all fitness levels. Recent upgrades to equipment were great. There are a number of trainers available to not only fit your fitness needs, but your style as well. Andy is great! I would recommend hi to anyone. He really pushes you to work hard to reach your fitness goals.
