A majority of my practice is providing outpatient services to families, individuals, and couples. In addition, I provide stress management classes to cardiac rehabilitation patients, clinical supervision to therapists seeking licensure, post-divorce parenting classes, and anger management classes. Programs, presentations, and talks are provided for companies and groups.

A. Insurance Information: You will need to call your insurance company before your first office visit to get pre-authorization for your sessions. The insurance company will give you an authorization number and the number of office visits allowed. Failure to contact your insurance company for this information prior to your visit would likely result in non-payment from the insurance company for the visit.
I understand that I am consenting and agreeing only to those services that the above named provider is qualified to provide within the scope of the his license, certification and training. If the patient is under the age of eighteen (18), I attest that I have legal custody of this child and am therefore allowed to initiate and consent for treatment.
