Transitions Healthcare's founders wanted to create a system that was more responsive to patients, with stronger continuation for physicians' treatment plans and closer coordination with hospitals. Our three principals come from the fields of Clinical Nursing, Finance and Nursing Home Administration. Their combined focus is on dedicating earnings to maximizing care and providing the right care to every patient.

This place is terrible please do not send anyone that you love hear they don't have no doctors hear they don't care at all they are just there to get paid it don't matter at all to them who lives and dies.

Doctors and the employees do not care about patients at this facility. Transitions they transitioned you from life to death with little care and draining your bank account. Please if you love your family dont send them here #1 in the country for bed sores and lack of care

Bad experience for 1 week. Biggest complaint was the nurses didn't seem to care and when you call for them, they came 15 to 30 minutes later. I had to call nurse station 3 times to ask them to go check on him because he was sick throwing up. He said all you heard all day and night was laughter while your laying there feeling like your dying. Shame shame shame

My family member didn't receive his medications, call lights never answered, patients left with their oxygen.
He literally needed to call 911. This place is a death trap. And their was an employee that smell like alcohol and his prescription is missing!

Do not send your family or anyone you know here especially if they have a trach or are unable to communicate. This place has killed my mom once then almost killed her again a wk and a half later from pure neglect. Beware!!!

I would not recommend this place to my worst enemy. The care is absolutely horrid. 39,27,26 minutes to answer a call light. The caretakers have no compassion for people. I could go on but what’s the sense? The state doesn’t care either. My advice is to keep a close eye on your loved ones if you’re forced to put them in this hell hole.

After visiting this facility, I must say the treatment from a staff member (Ashley) that I encountered was BEYOND unprofessional.
She was EXTREMELY rude, lazy and seem to be there just for a paycheck. She treated me like a piece of trash just because I had questions about my family members well being, etc. She was rude, unprofessional, loud and childish to say the least. I did bring it to another nurses attention who seemed to be very helpful. She did tell me her name was Ashley Pratt. I’m hoping her behavior would be corrected in the future. Perhaps you should reconsider your career path ASHLEY

The worst care ever!!!!! Must get kickback from drug sales. Really push the percocet so you dont bother them. Our cousin was here. Surly staff, no care, unrecognizable food. The worst. Get your loved ones out of there
