Welcome to Belmont Health + Rehabilitation Center, where we believe in enhancing quality of life through compassionate care. For more than 30 years we have served the changing healthcare needs of aging residents and others in the Madison area. We offer comprehensive long-term and short-term care options for individuals in need of 24-hour skilled nursing care or rehabilitative care after surgery or other illnesses.

Our rehabilitation program includes licensed physical therapists, occupational therapists and speech-language pathologists. Therapy is available seven days a week. Through all of these programs we're committed to a tradition of compassionate care we call Caring Above and Beyond. A close friend of mine worked at this facility and what I’ve heard about the faculty makes me sick. A nurse didn’t want to take the time to feed a dementia patient and when my friend questioned the nurse they said the patient “wouldn’t remember anyways”. The dementia patients get it the hardest considering this is a lot of the nurses attitudes. This is just one of the many examples of how poor the mindset of the staff is. If you care for your loved ones don’t send them here!!!

I'm the POA of a formerly homeless elderly man who has been a resident at Belmont for several years. I've found Belmont nursing staff to be caring of him and dedicated to providing him the best care possible. The facility is not a vacation designation but the love and concern of all the staff is comforting.

As a medically complex patient family member was doubled up in a room without access to anything. Had almost no needs met - medications weren't verified, they didn't even have life-saving epipens. Response times to room were 45 minutes plus for emergency calls, and had to call 911 multiple times on a barely charged phone, and got yelled at by staff for saving their own life. Extremely low care ratios.
Place smells, most residents are cognitively incapacitated or drugged so cannot complain. They put on a nice show at daytime when visitors are allowed and management is there, but when you're left

Just spoke with someone on the phone, I assume a receptionist. Very bad attitude. I do not recall her name but she was very rude. I would not recommend this place to anyone. Especially if they talk to people in that manner over the phone and in person.

This place used to be terrible about 10 years ago when my father stayed here. It was with hesitation that I went in to visit a family member 10 years later. I was quite pleasantly surprised. Most of the carpet and living rooms are new. It's well lit and well staffed. The food was decent nothing fancy nothing bad. There are still some pretty strong smells in the air that they don't a dress but most importantly it's really well staffed when we have been there. The one area that would be well improved as if they had some nice outdoor seating for more than a few people.

The quality of nursing care was terrible save for a few of the devoted nurses. The human recourses person would be better described as inhuman. Her treatment of residents was consistently terrible. Katie seemed to thrive on treating patients as less then human and a waste of her time. It always seemed she was annoyed or angered when approached by a patient. Also my medical records were deliberately entered incorrect. HIPPA laws were ignored in my care.

This nursing home is humble in appearance but they are absolute rock stars in patient care. The staff is loving, kind and helpful. The CNAs are exceptional. My father was happy and comfortable during his stay and we grew to really love and appreciate the staff. An undiscovered gem in my book.
