Dr. Jack Kotlarz specializes in the practice of Ear Nose and Throat problems and Facial Plastic Surgery. He attended Syracuse University, majoring in biology and psychology graduating summa cum laude. After serving in the Army for three years he attended medical school at the University of Illinois in Chicago.

DO NOT let this man touch your face. I am still dealing with the botched procedure 10+ years later. I would go into detail, but I've already wasted too much of my time/energy... I desperately wish I would have chosen a different doctor. I just hope I spare someone else from his dilettantish practice.

Called because I thought there may be a discrete in my bill was transferred to a new department and they did not answer. Left a voicemail and it took 2 days to get any response. When I finally got a call back I tried to explain the situation and kept getting cut off and told that what I was saying was incorrect before I had time to even finish. They only care about the money here.

I wasn’t impressed with my first appointment with this doctor. He was very short and didn’t even care what I had to say. I have a second appointment scheduled but I’m really thinking about seeking out another ent doctor. I wasn’t impressed at all by his professionalism. The staff appears to be friendly but not the doctor.

I have been seeing Dr Kotlarz and his team for over four (4) years now and I have been very impressed with his teams efficiency in getting vets in and treating them. Appointments are and can be made in advanced and have not been handled by Tricare in over a year. All Veteran referrals are now done through Optium.
The staff has always been polite and quick to assist and Dr K is always willing to listen to any concerns the patients have and he addresses them as quick as he is to listen. The office is always clean and calming.
I would highly recommend him and his staff to anyone who needs this type

Look like Tricare do not pay them(not really), Service Members cannot make an appointment before they receive the referral, as their "POLICY" they cannot make an appointment before they get the authorization from Tricare because they need to see the paper before make any appointment, not being important they will have to set up the appointment on the future, were Service Member can bring by hand the authorization or they for sure will have the authorization sent by Tricare. really bad service, I Called Tricare and Complain About it. Recently, Dr. Susan Aguiar was hired to be more manager, a “Karen”, than as a doctor in my opinion. Not only is she unprofessional, she disrespected me in front of the entire staff. Be wary of this lady. She has received numerous pharmaceutical kick backs in the past. Red flag! Doesn’t listen, doesn’t try to understand. Not to mention she made fun of people with mental disabilities. Honestly, this entire place was riddled with drapes over the windows. Hello, allergens/dust lurk in

Worse doctor and worse staff , ever. Staff never bothered to call me back and when I ask for Dr kotlarz medical assistant he answered "oh my God " on the phone. The lack of professionalism is unbelievable at this place. Seriously you're better off not having the surgery, get a second and third opinion
