Transitions Professional Center, LLC is an organization of psychologists and doctoral-level psychology trainees located in Portland, Oregon. We serve adults with serious, life-limiting medical illness, life-threatening illness, caregivers and those who are bereaved. Our office is in the process of relocating.

Please be patient with us as we make this change during already challenging times. We are currently accepting new patients for virtual visits only. When we are able to safely re-open for in person visits in our new location, we will update our information and reach out to our current patients and referral sources.
Services
Our goal is to meet the needs of our local community as well as to contribute to our broader professional communities. Portland, Oregon is a unique city - outstanding in its community building efforts. We aim to work with our remarkable hospital providers in Portland, and through strong consultation relationships, offer psychological services where needed.
Individual psychotherapy with psychologists specializing in palliative care, end-of-life, loss and bereavement is available at Transitions Professional Center. There are a variety if reasons that people become motivated to seek psychotherapy. Existential thoughts, insights, and concerns around life and death issues and the pursuit for meaning are common motivators to seek professional guidance.
Support groups are led by psychologists and psychology trainees. Support groups are available on an ongoing, open-enrollment basis and regular attendance is not mandatory. Number of group participants may vary from week to week and we will open additional group sessions if over 10 people. All support groups vary in duration and frequency with which they meet.
Our local psychologists will be available to present educational topics to Portland's clinical psychology graduate students, interns, and practicum students. In addition, we will provide talks and educational presentations for medical students and interested professionals in other related fields with interest in psychological aspects of end-of-life care.
We are eager to provide a space for palliative care, end-of-life care, loss and bereavement providers of all disciplines to join and learn of each others work and professional interests and activities.
