Services
Dr. Dawkins, Dr. Crittenden, Kelly Walworth, PA-C and Jordan Hintze, PA-C treat a variety of skin problems. These include skin cancers, pre-cancers, psoriasis, acne, warts, sun damage, rashes and other skin conditions. We provide treatment for skin cancers, aging, fungus, warts, and sun damage. Treatment is also provided for hyperhidrosis (excessive sweating) with miraDry and Botox therapy.
Over eighteen years experience in beauty and aesthetics has taught me that each person has a unique desire for his or her looks. Having been mentored by a plastic surgeon with meticulous detail for beautiful outcomes has been a great asset. Listening to your unique areas of concern is where it begins.
Using thermal energy, this procedure targets and eliminates sweat and odor glands located under your arm, easily reducing your underarm moisture. Hyperhidrosis, excessive sweating, is a condition that is much more common than you might think. Millions of people are plagued with this inconvenient and often embarrassing condition.
Minors must be accompanied by a parent or legal guardian for the initial new patient office visits, or they will be asked to reschedule their appointment. If the patient is 16 or 17 year old, they may be seen for follow up visits. If your insurance plan requires an authorization form your Primary Care Physician (PCP) we will need to have a current copy of your authorization in our office before we can schedule your appointment.
To assure your billing record contains current and accurate information, we will make a copy of your health insurance card at your visit. Our doctors participate in several PPO and HMO insurance plans. We will file claims for patients in these plans. You are responsible for payment of any deductible, co-insurance, or encounter fee (co-pay) at the time of your visit.

Horrible experience if you don't have insurance or insurance card with you they don't even want to look at you. Don't have your nurse try to call them either cause they don't want to talk to them either. They don't want to hear your story or situation or anything even if your a heart transplant patient they want talk to your transplant team either

I waited two months for this appointment that I desperately needed, as. I have been in pain from my skin issues. I have been going to my primary in the mean time but they can only do so much. I used to go to this doctor growing up and she is fantastic. However when I scheduled staff said they took my insurance.
I took off work, only to arrive and find out they do not take my insurance. If you have state employee insurance they probably can't see you.

Although I have gone to Dawkins on occasion for years, today I was told it would be six weeks before I could get an appointment to have a couple of warts froze. This is not the first time I have been asked to wait for over a month or two for a simple procedure that takes ten minutes. What good is a clinic if it takes six weeks to get a ten minute appointment?
