Services
LifeSpan Care Management, LLC (LCM) is the premier provider of health coordination and advocacy services for individuals and families encompassing the entire lifespan. Highly skilled nurses guide and advise families on the care of loved ones and provide assistance to assure quality medical care and access to appropriate health services.
All services start with a thorough evaluation of the client and family situation. A written report, documenting the results of the interview, record review, etc., includes a Plan of Care. Advocates for clients at care conferences in hospitals, subacute or other care facilities. Refers and quality-controls referrals to hands-on providers as needed, such as home health agencies, specialty physicians, Assisted Living Communities, Nursing Homes, etc.
