Charlene Manor Extended Care Facility has been part of the Greenfield community since 1987, providing short-term rehabilitation, long-term skilled nursing care, and specialized dementia care in the Pioneer Valley. We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals.
Our dedicated core team includes nurses and nursing assistants, full time nurse practitioner, rehabilitation therapists, activities staff, a dietitian, consulting physicians, and our Medical Director.These specialists, along with our entire staff, work together to provide a place that feels like home.

Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician.
Our short-term rehabilitation unit is ideally sized for personalized care, with just 41 beds in private and semi-private rooms. Because we have a certified dementia care unit, all of our staff are specially trained in caring for individuals living with memory impairment, providing compassionate, respectful care to assist patients in returning to their prior living situation.

I am only giving 5 stars because of this one aid that I truly am grateful for. She went above and beyond for our family member who unfortunately is no longer with us. Melissa bacigalupo made us and our family member comfortable for the short time we were there. I had a difficult time with a few other aids who work there, just trying to get clean sheets or even fresh water was a hassle for them. Melissa was always available for anything we needed or any questions we had. We are truly and forever grateful for her help and sincerity in our time of need.

I had a family member in local "for profit" nursing facility for 5 years before moving them to Charlene Manor, a non-profit facility. I can tell you with 4 years of first hand experience there, that Charlene is the best local nursing home hands down. None are perfect, nor can they be and keep their doors open. They never once failed to meet any request we ever had in regard to the care of our loved one.

I recently had a loved one at this facility to get help walking again. They said they were able to help bariatric patients but lacked the equipment. So my loved one sat in this place for 6 weeks not receiving adequate help. Several times he would press his call light for a nurse to come in, however it would take 30-45 min for any kind of response. I kept hearing about short handedness but that's not my loved ones problem. When a patient sits in their own urine and feces because they can't hold it anymore, that's a HUGE PROBLEM!!! One of the CNA'S even said there were only three staff to 41 beds

MY FRIEND WAS ADMITTED TO CHARLENE MANOR IN GREENFIELD MA AROUND FEB 22ND AFTER A FEW WEEKS CHARLENE CALLED ME TO INFORM ME THEIR AIDE LEFT MY FRIEND A HIGH FALL RISK IN THE BATHROOM ALONE AND SHE FELL A FEW WEEKS LATTER SHE CALLED TO TELL ME SHE FELL OUT OF BED AGAIN I HAVE FOUND SHE WITH HER MOUTH DIRTY MANY TIMES ONE EVENING I WENT TO VISIT HER WITH ANOTHER FRIEND AND HER CLOTHS WERE BEYOND FILITHY HER HANDS WERE LITTUERLY BLACK FROM THE DIRTY OLD WHEEL CHAIR THEY GAVE HER BECAUSE MY FRIENDS WHEEL CHAIR SOME HOW CAME UP MISSING WHICH THEY SAID WOULD REIN BURSE HER NOW IN THE MIDDLE OF JUNE STILL
