We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals.
Our dedicated core team includes nurses and nursing assistants, full time nurse practitioner, rehabilitation therapists, activities staff, a dietitian, consulting physicians, and our Medical Director.These specialists, along with our entire staff, work together to provide a place that feels like home.

Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician.
We start planning for discharge upon admission, creating personalized treatment plans that implement physician's orders to meet personal goals. Our 24-hour skilled nursing teams provide the care and attention needed to maximize recovery, and a family meeting is held within 72 hours of admission to review expectations and address any concerns.
At North Adams Commons, we focus on the individual, rather than the illness. Our residents and patients receive personalized around-the-clock nursing care and supervision as needed, including assistance with activities of daily living, such as dressing, bathing, eating, and getting around. Orthopedic rehabilitation for joint replacement, knee replacement, hip replacement, fractures, and degenerative joint diseases.

Our niece and my sister in-law’s daughter, was a patient for several months prior to her death. The staff, from reception to nursing to housekeeping to facility maintenance, was at all times, diligent, professional, and caring. They went out of their way to be supportive during a very difficult time for our family. We are thankful and proud to have such a fine facility as a part of our community.

Well same things at sweet brook rehabilitation and nursing center ... i do not like that place ..... reason ... nurse is doin same thing not have skilled, nice, freindly and helpful.....i got angry at sweet brook ... long story ... i dont want any people go there at sweet brook....... NURSE IS REALLY MEAN.. AND RUDE TALK ... ALSO LAZY AND DONT WANT AND DOIN THEIR JOB ...

Very unskilled nurses seems the CNA,s make you much more at peace then the nurses although just a hand full ,Wich is to many. We already have a society where the pharmaceutical agencies control prescription medications we don't need robotic nurses thinking in the same kind of lines, there has to be some compation with out bringing it home with you, like maybe human !!! Everyone who stays here ,our lives here has one time or another has given up lot. The three stars are for the physical therapy ,very respectful and knowledgeable and skilled it's unfortunate that I can't give a better review ,the
