In the late 1800's and early 1900's, Wisconsin chose to develop a series of county hospitals and two state hospitals. In 1898, Trempealeau County officials made a decision to build a county "asylum" and start a farm to provide service to mentally ill, intellectually disabled, and alcohol and drug dependent persons.
Trempealeau County Health Care Center is a county-owned facility with roots beginning in 1898. Trempealeau County Health Care Center has continued to expand throughout the many years of operations to bring about a comprehensive system of care. Using a recovery and person-directed care approach the Trempealeau County Health Care Center, focuses on assisting persons to be prepare for community living by promoting the individuals well-being.
A primary service provided at Trempealeau County Health Care Center on the IMD-NF units includes a focus on safety using an interdisciplinary treatment team approach. We provide two secured IMD-NF units that support an individual's safety while receiving treatment.
Additionally, an unsecured unit and Gero-Psych Stabilization Unit within the IMD-NF licensure allows for individuals who experience less struggles with safety to receive treatment with close access to a more supportive environment, if needed.Once an individual has completed our treatment program they are discharged to one of our community-based placements or return to the community to continue their recovery in a less structured setting.

Our comprehensive care approach allows us to individualize each person's care to support his or her progress. We offer several different treatment modalities that continue to be developed to meet the specific resident's needs. Psychiatrists on staff in addition to medical providers to address the individuals physical and mental health needs.
Pigeon Falls Health Care Center is a skilled nursing facility located in the rural community of Pigeon Falls, Wisconsin. Built in 1979, with major remodeling completed in 2010, it has long enjoyed strong community support. The facility sits atop a hill in the community, giving it a view of the serene agriculture that surrounds the building.

A family member is currently in this facility. It is located in a beautiful, peaceful setting. They have a walking track, gazebo and picnic tables, a pretty water fountain, beautiful, well kept facilities, good programs, friendly, dedicated staff. My family member says she feels very safe there and feels good about their programs. She said her meals there are very healthy, well prepared, and delicious. I am relieved that she is there and getting quality care to get her back on track.
