In order to request an account, you must be at least 18 years of age, be an existing patient and have a valid email address. For your protection, our portal uses identity verification questions as part of the account request process. To request a portal account, for a child age 17 or younger, please call the Health Information Management department Monday through Friday, between 8:00 am and 4:30 pm at (405) 271-2310.

Legal guardians of patients age 18 and older can also request by calling Health Information Management for assistance. To request a child's record, please complete a Medical Records Release Form. Print the form from Request Medical Records webpage, request a form by emailing Medical Records Department, or call (405) 271-2374.
