Services
The practice opened in September of 1991 in Kentwood, Michigan. It was founded by Drs. Bruce and Susan Baker with an emphasis on a caring, thoughtful, proactive approach to medicine. Our desire to listen to our patients often leads to a more accurate diagnosis in less time than just ordering test after test.
If you have not already received these forms, they are available for download (see below). If the patient is a minor, a parent or legal guardian must accompany him or her to the initial office visit. Due to Federal Privacy Regulations we regret that we are unable to answer medical questions or address billing concerns via general e-mail.
Kentwood Family Physicians is excited to be the only practice in Grand Rapids, and West Michigan that offers i-Lipo, the intelligent, non-surgical alternative to liposuction. What is a deductible? A deductible is a set out-of-pocket amount that you must pay before your insurance company will pay anything. Why is the practice named Kentwood Family Physicians, when it is not located in Kentwood? We started in Kentwood over 20 years ago and kept the name when we moved to a newer, larger office and have kept the name since that is how most of the patients know us.
