I have recently updated the website with further details that I hope will assist you with this transition. Additional information about records release is provided on that page as well.
Due to the large volume of records being transferred, please allow up to 30 days for me to complete preparation and transfer of your records. The process on our end requires manually compiling the various parts of your electronic chart into a new document for release. If you have scheduled a visit with another doctor within 2 weeks, please advise and we will make every effort to expedite your request.

Your medical records can be released only with your written consent. Please begin the process by downloading and completing the above form, or by obtaining a similar form from your new doctor's office. We would like to complete records transfers electronically whenever possible, and there are several options to do this without charge.
Driveway entrance is immediately adjacent to the Enterprise car rental building and easy to miss. We are a small, single physician practice emphasizing personalized and patient-centered care. Dr. Poole provides care for adult patients with skin cancer and other skin disorders with the support of her medical assistants, Anita and Kristie.

Please try to arrive 5 minutes before your appointment time, so that insurance information can be verified and personal information updated in your file if needed. In preparation for your appointment, it is suggested to limit use of moisturizers and fragrances on the day of your visit and, if possible, to wear no make up if you wish to have a skin exam.
It is the policy of our practice that payment is due at time of service unless other arrangements have been made in advance. If you are covered by medical insurance, please provide your complete and up to date policy information and we will attempt to verify your coverage as a patient courtesy.
We will be happy to file our charges with your primary insurance carrier as a service to you, and will also file to a secondary insurance that is recognized by Medicare and has the ability to receive electronic filing.

Dr. Stein is an accomplished graduate of Ohio State University College of Medicine and the Cleveland Clinic Foundation who sees patients for Mohs skin cancer surgery in referral from local dermatologists, including Dr. Johnson and ARNP Dana, as well as many others. She has been a fixture in our community for over 20 years and has a stellar reputation with both her colleagues and her patients.
