The mission of The Healthcare Consortium is improving access to healthcare and supporting the health and well-being of the people in our rural community. Anyone who lives or works in Greene County and lacks access to a computer and/or the internet may call 518-822-9600 to be assisted with completing online vaccine registrations by staff at the Healthcare Consortium.

No more than three visitors (including children) will be permitted in the client meeting room at one time. The Helpers for Health Project staff can help you to GET AND STAY HEALTHY. Individuals and families in Columbia and Greene Counties can get help with things like understanding how to use health insurance, finding a doctor, scheduling medical appointments, and arranging transportation.
Services
The Healthcare Consortium is a rural health network made up of your friends and neighbors from the health care, business and consumer sectors. It is one of 34 such networks throughout New York State and receives funding from the New York State Department of Health, Office of Rural Health, and through public and private resources and donations.
The Columbia County Community Healthcare Consortium, Inc. ("the Healthcare Consortium") is a not-for-profit corporation established in 1998 to create a network of health and human service providers serving the rural community in and around Columbia County.

It is governed by a multidisciplinary Board of Directors, which includes representatives from county government, county public agencies, our local hospital and its network of providers, the mental health, substance abuse, and disabled populations, the clergy, business interests and healthcare providers and consumers.

Staff in the Navigator Program assist individuals, small businesses, and small business employees in Columbia and Greene Counties with signing up for affordable health coverage, including Medicaid and Child Health Plus, through the New York State of Health: the Official Health Plan Marketplace.This program is designed to provide door-to-door, non-emergency medical transportation to Columbia County residents.
