Our dedicated team plays an integral part in the success of our patients at Lake Park of Madison Nursing and Rehabilitation. The members of our skilled and diverse therapy team have decades of combined experience with specialties in orthopedics, spinal treatment, neurological treatment, wounds, geriatrics, wheelchair positioning, restraint reduction, swallowing, cognitive staging and urinary incontinence.
At Lake Park of Madison Nursing and Rehabilitation, our residents are treated to a full-service therapy regimen which is personalized to our patients' individual needs. Whether recuperating from surgery or regaining independence following an illness, each of our patient's personal needs are taken into consideration when coordinating a therapy plan.

It's not the place it's the people. Athena, Rhonda, April, Josh, Kara, Vicky, Tyaunie and Lynn are the names that come to mind right away. Each of them is awesome in their own way and have been extremely helpful in getting my mother-in-law into the facility and cared for. We are extremely grateful for all of their assistance and loving and caring attitude.

My visit with my mother was great. It appears that she's being taken care of in a caring manner. The reason for my rating is that upon our departure there was no one at the front desk for security purpose. There were two ladies that said they had been waiting for a while to be escorted to see there love one as my family was also signing in. The young lady returned having gone out to get something to eat. There should be some coordination so she can leave while someone sit in not leaving the entry unattended. The concern is that ANYONE could have entered the building with not so good of intentions

The place is bright and clean, decorated pleasantly and it is very peaceful!
However, my father was in a lot of pain when I last visited him. He was prescribed Lortab but we were told they just give him regular Tylenol until the pain gets bad even though he's prescribed it 4 times per day. He has taken it for years prior to his being in a home, so regular Tylenol does nothing to touch his pain. We would like to see him upbeat instead of suffering. I hope this can change.

Please beware of placing your loved one in this facility. Incompetent, non caring, unprofessional employees. Nursing staff has a lot to be desired. Never assisted my mother in putting away her belongings when she arrived especially since no family member could go in facility due to covid-19 regulations in place. Staff is definitely taking advantage of that. Eat part of my mother's fruit out of an unopened fruit platter and placed the trash pieces in her bag and left in the room. Staff is not in uniform. All doors remained closed to patient rooms. There could be an emergency and no one would know

After moving my sister from a terrible Nursing Facility, Lake Park of Madison is a godsend. They keep her clean, they keep her well fed, without any bruising or concern for her health. Every time I go the facility is clean and brightly lit and full of visitors. I recommended to anyone needing to find a nursing facility or a loved one.
