Home Health & Hospice Care has been a provider of home care and hospice services since 1883, making us the region's most experienced Visiting Nurse Association. Serving 25 communities in southern New Hampshire including Manchester, Nashua, Derry, Salem, Merrimack and beyond, we provide traditional home care, inclusive of nursing, rehabilitation, therapies, medical social work, and licensed nursing assistants.

After suffering severe pain from complications of total knee replacement while own PCP was on vacation and my surgical nurse could not schedule me to be seen for 3 to 6 days, Kristi, my Home Health and Hospice physical therapist, insisted that I must be seen. Since no one else could see me, she recommended Immediate Care nearby. Eventually, I was sent to the surgeon for x-rays, then to the hospital for an ultrasound. I was diagnosed with gout and a blood clot and started on medication immediately. Had it not been for Kristi's insistance that I be seen that day, who knows what would have happened
