Chestnut Hill takes an interdisciplinary approach when it comes to managing the resident's short term or long term healthcare. Each resident is assessed by an experienced healthcare team who then develop a plan of care individualized to meet that resident's needs. This approach ensures each resident is participating in a plan of care that will enable a smooth transition back to their routine activities of daily living.
Chestnut Hill offers a comprehensive rehabilitation program and services, and optimum patient care to achieve the highest functional mobility, independence and quality of life for the patients we serve. Chestnut Hill has qualified, professional nursing staff on-site 24 hours a day, 7 days a week to provide the skilled nursing services as ordered by your attending physician.

Our Rehabilitation Team includes the physician, rehabilitation nurse, physical therapist, occupational therapist, speech therapist, social worker, case manager, recreational therapist, psychologist and dietician who work together to develop an individualized treatment program for each patient to restore functional mobility, improve balance, endurance and physical strength.

The staff of Chestnut Hill Residence Assisted Living is committed to preserving your independence, privacy and dignity in a beautiful home-like environment. Each resident has a care plan which describes personalized care to meet a resident's needs. Regular visits by a licensed nurse are provided. Hands-on personal care is also available.

My dad was at this nursing home due to health and my step mom passing. My review is in the staff; not so much the facility. Every time I visited his day to day nurses and staff did show him lots of care. A sincere thank you to Renata, Fanny, Thelma, Joanne. The facility in general is basic, but I'm told complete care bought them out and they are working on much needed improvements. It can take long at times to get thru the phone system. Beds need updating, my dads was crank style for a while. With any nursing home the family must still be involved and attentive to the patient.
