Since its founding in 1964, the Chicago orthopedic doctors of Midland Orthopedic Associates have been dedicated to providing our patients with the highest standard of care. We strive to provide high quality, cost-effective, compassionate care with our goal being to exceed our patients' expectations.
You should bring a photo ID, a copy of your insurance card, a referral if needed with your insurance, a copy of your current medications, and a copy of any tests that you have had ie Xrays, MRI, CTScan. Please arrive 10 to 15 minutes early to allow time to complete paper work if you have not already done so online.

The front Desk staff were very patient with me. My appointment was a little disorganized, missing certain paperwork but they helped me out EVERY step of the way. The physical therapist are all GREAT, welcoming, professional and make your experience fun during recovery. They also work well with my busy schedule.

Dr. Booker is great but you have to get past the evil intake lady at the front desk before you get to him. in reading reviews I see others had the same problem. I do not know how that woman became someone who's mind thought she could work with the public .she has the nastiest attitudes of anyone I have ever seen. I have had more than once to refrain myself from cussing her out..
My daughter-in-law died July 10th and I have an appointment July 12th.
O I kept the appointment because dr. Booker is so busy that is hard to get an appointment with him it seems that the wrong doctors name was listed

Customer service is terrible at front desk. The older ladies are not attentive to the needs of injured patients. Considering that mostly all of their patients have just had injuries or surgeries that’s preventing them from walking independently, there should be some type of training on patient service and how to serve injured patients. I was told to “do the best I can” after informing one of the ladies that I won’t be able to come back and forth up here to the desk when told to fill out paperwork and bring the paper work back to the front desk. Dr Heller is a fine doctor. HOWEVER! The staff is absolutely horrible and bait and switchers. I called to make my appoint and was in formed they needed a credit card on file so that they can take the balance after the insurance. I asked if there is an option to set a payment plan and was told yes. With that information I went ahead and scheduled my appointment. I was told the same information when I arrived at my appoint and that I would be able to set up a payment plan. They also told me they would send out a bill for the services which NEVER happened. The staff at this location is VERY rude with POOR customer service. Due to the lack of communication on thier part, twice I have taken a half day off from work to go there only to get there and be told I can't be seen. Had I been informed of what I needed PRIOR to my arrival (which should have been at the time a made the apointment a WEEK in ADVANCE), I would have done so. Furthermore, to the told by a "staff member" that they have well over 500 patients and it's not their job to know what a patient needs, paients have to take some responbility for their own care and its not your fault I have HMO
