Annie Swartz, RN, BSN, has been a Registered Nurse since 1992 and has over ten years of experience in the home care industry. Little Silver Home Care, LLC, is a consulting agency that assists clients to choose a Licensed Health Care Service Firm to place qualified caregivers in their home. Annie will interview the client and develop an individual plan of care to understand their needs at home.
We utilize the nursing process to help clients and their family members figure out home care in order to maintain a heallthy and safe home environment. Navigating the home care process can be stressful. We will advocate for the client and their home care needs. The nurse can discuss with: physicians, nurses, social worker, physical therapists, occupational therapists and speech therapists to evaluate the client's goals.
