Services
An Innovative Care serves residents who are ambulatory, semi-ambulatory or non-ambulatory and who may also be of advanced age, physically disabled, terminally ill, or diagnosed with Alzheimer's or a related dementia. Each resident unit is furnished with a bed, dresser, closet/wardrobe, and has plenty of space for residents to personalize their room with belongings.
We are partnered with several Managed Care Organizations (MCO) to accept Family Care, and Partnership programs. Family Care is a Medicaid long-term care program for frail elders, and adults with physical, developmental, or intellectual disabilities. Our MCO partnerships include: Community Care, iCare, and My Choice Wisconsin.

Myself along with the majority of the neighbors to this facility are fed up with this facility. Tonight 3-1-2021, 3 different police forces, (Kenosha County, Kenosha City, and Pleasant Prairie) yes, all 3 at the same time, were there several times throughout the day. Employees constantly speed down the road. There is NO LONGER on street parking in front of the facility. And we have been informed that this establishment has been complaining about other neighboring properties. Being a neighbor I wouldn't recommend this facility to anyone after what myself along with many other neighbors see. And

Worked here and it isn't what the picture portrays. They dont treat staff how they should, management doesn't know how to run things properly and the residents are not treated how they should and they do favoritism. Definitely not a good place to work or to put your family! Things are alot different behind closed doors.

Beautiful, beautiful home, backyard is amazing nature is a huge WOW factor!! Peaceful and serene at it's finest. I couldn't ask for anything more. The manager here is the best and always makes sure all residents care and needs are always met,kind hearted and very loving. All caregivers give 100% and make sure all residents are well taken care of,they are always aware of each individuals cares and perform all daily duties in a timely manner. I am very very happy with this home and all they have to offer,it's a 5 star with no complaints.
