Prior to scheduling an appointment, confirm with the doctor's office staff that the insurance plan you are enrolled in is still accepted by the physician.
All patients must bring a Driver's License or State ID, a valid insurance card, and a list of medications. Co-Pays are due at the time of service. If you are unprepared to pay your co-pay you will be asked to reschedule your appointment. If you arrive very early for your appointment, we will try to see you sooner if possible.
