We do this by designing a customized treatment plan that will enable patients to restore mobility and increase strength, as well as decreasing pain and/or swelling. Services
We will schedule follow-up visits and notify physician's office of absences / cancellation / rescheduling of patient. Assess vocational needs to determine what modifications may be necessary to enable employee to return to work. Will work with case managers to find a way to get employee back to work at least part-time (not feasible in some industrial situations).
In an effort to provide additional service options to our clients, we have made all patient forms available to you for download. Please note that all new patients will need to download the New Patient Forms and the Privacy Notice. Other available forms pertain only to specific injuries / problems and you only need to download those that apply to you.
So that all parties may be served, every effort will be made to accomodate various schedules. Please let us know of any time or scheduling challenges and we will attempt to arrange other appointment times as needed. Out of courtesy to others, we ask patients to notify us if they are going to be late or if they are unable to attend a scheduled appointment.
