The New Patient Paperwork is designed to inform us about the pain and discomfort you are feeling prior to your initial examination. Also, share with us a little history and information about your overall health.

We speak to every patient post-operatively in the recovery room. Additionally, we make every attempt to speak to the family and friends that accompany you for your procedure. Unfortunately, sometimes we are not able to speak your family and friends; frequently, they are not in the waiting room. It is common for patients under the influence of anesthesia not to remember post-operative conversations.
