Studies show that patients get better when they are home, replacing long hospital stays. Yet, we all know the transition from hospital to home can be an anxious and frustrating endeavor. Our Intake Department works closely with Hospital Discharge Planners and Insurance Companies to help plan a smooth and quick discharge process.
Once the patient is home we provide knowledgeable and compassionate nurses and therapists to teach patients their home care, and give them the confidence they need. We also have skilled nurses to provide long-term care for those who need help with nursing needs and ADLs, allowing them to be cared for in the comfort of their own home.
